What is Oxidative Stress?
💡 Oxidative stress occurs when reactive oxygen species (ROS) production overwhelms the body's antioxidant defences. In reproductive medicine, it damages sperm DNA, impairs oocyte mitochondrial function, disrupts embryo development, and reduces endometrial receptivity. It is elevated by varicocele, smoking, obesity, and advanced age.
Oxidative stress occurs when the production of reactive oxygen species (ROS) — unstable molecules including hydrogen peroxide, superoxide, and hydroxyl radicals — exceeds the body's antioxidant defence capacity. In reproductive medicine, oxidative stress damages sperm DNA (raising DNA fragmentation index), impairs oocyte mitochondrial function, disrupts embryo development, and compromises endometrial receptivity. It is elevated by varicocele, infection, smoking, obesity, advanced age, and environmental toxins. Antioxidant supplementation is the primary strategy for reducing reproductive oxidative stress.
🇮🇳 India Context: Oxidative Stress is widely assessed and treated across major Indian fertility centres including Chennai, Mumbai, Bangalore, Delhi, and Hyderabad.
Key facts about Oxidative Stress
- ROS are unstable molecules: hydrogen peroxide (H₂O₂), superoxide (O₂⁻), and hydroxyl radicals (•OH)
- Normal physiological levels of ROS are essential for sperm capacitation and acrosome reaction
- Excessive ROS overwhelm antioxidant defences (glutathione, superoxide dismutase, catalase)
- Sperm are uniquely vulnerable — high PUFA content in membranes, minimal cytoplasmic antioxidants
- Raises sperm DNA fragmentation index (DFI): DFI >25% associated with failed fertilisation, poor embryo quality, higher miscarriage
- In women: oocyte mitochondria produce ROS during energy generation; excessive ROS impair ATP production and spindle integrity
- Elevated by: varicocele (testicular hyperthermia → ROS), smoking, alcohol, obesity, infections (leucocytospermia), environmental toxins
- Measured indirectly by DFI testing; ROS assays available in specialist andrology labs
Why Oxidative Stress matters in fertility
Oxidative stress is the unifying mechanism behind many modifiable male fertility impairments. Elevated DFI (>25%) on SCSA or SCD testing is the clearest clinical indicator. Treatment hierarchy: (1) identify and treat the source — varicocele repair, lifestyle modification, infection treatment; (2) antioxidant supplementation for 3+ months; (3) retest DFI and repeat semen analysis before IVF/ICSI.
